These Terms of Service (“Terms”) govern your access to and use of healthvitalflow.com (the “Site”), operated by Form and Momentum Media (“we”, “us”). By accessing or using the Site you agree to be bound by these Terms. If you do not agree, please do not use the Site.
1. Informational purpose only
All content on the Site is provided for general informational and educational purposes. It is not medical advice, diagnosis or treatment, and it is not a substitute for consultation with a qualified healthcare professional. Use of the Site does not create a doctor–patient relationship. Never disregard professional medical advice, or delay seeking it, because of something you have read here. In an emergency, contact your local emergency services immediately.
2. Self-assessment tools
Any quiz, questionnaire or self-assessment offered on the Site is an educational instrument. It is not a validated diagnostic test, it does not diagnose or exclude any condition, and its results carry no clinical weight. Results are generated in your browser from the answers you select and are not reviewed by a clinician.
3. Advertising, sponsorship and affiliate links
The Site carries advertising and contains affiliate links. Sections that are paid placements are labelled as such. If you click an affiliate link and complete a purchase, we may receive a commission at no additional cost to you. We are not the seller of any product referenced on the Site, we do not process payments, and we do not ship or fulfil orders. All purchases are transactions between you and the merchant, governed by that merchant's own terms, pricing, shipping policy, refund policy and guarantees. Any question about an order, a charge, a subscription or a return must be directed to the merchant.
4. Third-party sites
The Site links to websites we do not control. We are not responsible for the content, accuracy, availability, products, services or practices of any third-party site, and inclusion of a link is not an endorsement of every statement made there.
5. Acceptable use
You agree not to: use the Site for any unlawful purpose; attempt to gain unauthorised access to the Site or its infrastructure; interfere with its operation; scrape, copy or republish substantial portions of its content without permission; introduce malicious code; or misrepresent the Site's content as medical advice from a licensed provider.
6. Intellectual property
Text, layout, design and original graphics on the Site are owned by Form and Momentum Media or used under licence, and are protected by applicable copyright and trademark law. Product images and brand assets belong to their respective owners and are used for identification and editorial purposes. You may quote short excerpts with attribution and a link; systematic reproduction requires prior written permission.
7. Disclaimer of warranties
The Site is provided “as is” and “as available”, without warranties of any kind, express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, accuracy, or non-infringement. We do not warrant that the Site will be uninterrupted, error-free or free of harmful components, or that any information on it is complete, current or applicable to your circumstances.
8. Limitation of liability
To the maximum extent permitted by law, Form and Momentum Media and its officers, employees and contributors shall not be liable for any indirect, incidental, special, consequential or punitive damages, or for any loss of profits, data, goodwill or health outcome, arising out of or in connection with your use of the Site, its content, or any product or service referenced on it — whether based in contract, tort, negligence, strict liability or otherwise, and whether or not we were advised of the possibility of such damages.
9. Indemnification
You agree to indemnify and hold harmless Form and Momentum Media from any claim, demand, loss or expense (including reasonable legal fees) arising from your misuse of the Site or your violation of these Terms or of applicable law.
10. Age requirement
The Site is intended for adults aged 18 or over. It is not directed at children, and we do not knowingly collect information from them.
11. Changes and termination
We may modify, suspend or discontinue any part of the Site at any time, and we may revise these Terms. The date at the top of this page reflects the latest revision. Continued use after a change constitutes acceptance.
12. Governing law
These Terms are governed by the laws of the United States and of the state in which our business is registered, without regard to conflict-of-law principles. If any provision is held unenforceable, the remaining provisions remain in full force.
